Jeremy Martin

CO-CEO AND CHIEF INNOVATION OFFICER

Jeremy has spent more than 25 years leading healthcare organizations across the senior, post-acute and community-based healthcare spectrum. He has worked at virtually every level of the industry, starting as a trainee and eventually working into his current role as founder, owner, and operator of companies in seven states.

In addition to co-leading the Executive Team, where he works to ensure both adherence to core principles and a commitment to fresh ideas, Jeremy also continues to steer the development effort, where he remains ever-passionate about innovating for the good of the patient.

Jeremy enjoys traveling with his wife, daughters, and dogs and hopes to pick up his clubs again someday.

Antonio “Tony” Gamboa, MD, MBA

CO-CEO AND CHIEF MEDICAL OFFICER

Tony is a physician executive who has taken healthcare service organizations from startup through exit, backed by both private equity and venture capital. He founded Elite Patient Care, which served more than 350 facilities, and led its integration into Curana Health, where he was President and later Chief Growth and Innovation Officer. Tony holds an MD from UT Medical Branch and an MBA from UT Austin, and is board certified in Internal Medicine and Hospice and Palliative Medicine.

At CareM, Tony shares enterprise leadership with Jeremy, sets clinical direction across all service lines, and is accountable for physician services.

Tony and his wife have three kids and a dog. On weekends, you'll find him at a sporting event or on a college tour.

Shonu Gandhi, JD

CHIEF GROWTH OFFICER

Shonu is a healthcare strategist who has tackled payment and payer challenges from every side: payer, provider, medical device, pharma and policy. She has held leadership roles advising payers at McKinsey & Company, leading clinical program development and reimbursement relationships for the University of Texas, Austin and leading market access and clinical for the world’s largest women’s health technology company, Hologic. She holds a JD from Harvard Law School and a BA from Yale University.

At CareM, Shonu leads growth across all business lines, including payer contracting, the value-based care program, new care products, and the policy agenda at the federal, state, and local levels.

Shonu and her husband live in Austin with their two sons. You can find them playing soccer or looking for treats.

Michael Hoskins, MHA, MSAcc

CHIEF FINANCIAL OFFICER

Mike brings over 30 years of healthcare experience to the team with executive leadership roles in both acute and post-acute care. Due to his varied background across operations and finance, Mike heads up financial operations, ensuring that the team always has the data to make quick, informed decisions.

Mike also assists in the development effort by contributing expertise in financial diligence.

Mike and his wife have two children and are enjoying their newly-empty nest.

Renee Kohr, MA/CCC-SLP

CHIEF INFORMATION OFFICER

Renee brings with a wealth of experience in healthcare operations and strategic development, with a knack for project management and organizational leadership. She is known for building strong partnerships to improve healthcare outcomes and streamline operations.

Renee started her career as a Speech-Language Pathologist and steadily moved into leadership roles at Corus Health, her last post as as Executive Director. Most recently, Renee was Vice President of Operations at Ensemble of Albuquerque LLC, where she led initiatives to enhance care quality, lower costs, and boost patient satisfaction through innovative population health management strategies.

Outside of work, you might see her spending time with her family, trying out a new recipe, or listening to an audiobook.

Alfred Macdaniel, JD

GENERAL COUNSEL & CHIEF COMPLIANCE OFFICER

Alfred is an attorney and former public accountant educated at the University of Texas at Austin. He has held executive roles at two medtech companies and two venture capital firms, including CFO, Chief Compliance Officer and General Counsel at TVA Medical and Chief Strategy Officer and General Counsel at Acessa Health. He serves as a director of several startups and previously advised healthcare service companies and served as interim CFO of a diagnostic software company.

At CareM, Alfred leads legal, compliance, and capital formation.

Alfred and his wife have a daughter and a rambunctious Bernese Mountain Dog named Thea.

Angela Sells, MSN, RN

CHIEF CLINICAL OFFICER

Angie is an accomplished nurse executive with a distinguished career in patient-centered care and healthcare leadership. Her rich nursing career has offered her the opportunity to provide nursing care for all ages, in all settings of care, across the US and internationally. As a healthcare leader, Angie is known for her expertise in driving operational excellence, fostering a culture of collaboration, and implementing innovative strategies across the continuum of care to drive improved clinical outcomes and enhance the patient experience. With a master’s degree in nursing executive leadership, Angie remains committed to mentoring the next generation of healthcare leaders and caregivers in advancing community-based healthcare.

Angie enjoys spending time with her husband and two dogs in their amazing New Mexico home, traveling with her three children and three grandchildren, walking her labyrinth or gardening in her greenhouse.
